Causes

Open or short circuit in wiring harness
Faulty fan relay

How to Fix

Check the fuse corresponding to the fan circuit
Make sure the fan relay is working properly
Check the power at the motor connector with the ignition on
Check the motor itself by applying power directly
Visually inspect the wiring harness for damage.

Replace fan motor or circuit components as necessary
