Causes

Short circuit of the control wire to power (+12 V)
Open ground circuit
GPCM module malfunction
Damage to wiring or connectors
Internal ECU fault

How to Fix

Read OBD-II codes and capture a freeze frame
Inspect the wiring and connectors of cylinder 5 spark plug and GPCM module
Measure the voltage in the spark plug control circuit
Check the ground circuit for open circuit
Check signal wire for short to power
If serviceable - diagnostics and replacement of GPCM or ECU
