Causes

Short circuit of the control wire to power (+12 V)
Fan ground circuit open
Cooling Fan 1 Relay Malfunction
Damage to wiring or connectors
Internal ECU fault

How to Fix

Read OBD-II codes
Check the operation of the fan with a cold and hot engine
Measure the voltage in the fan control circuit 1
Check relay and connector
Check the control wire for short to power
If the circuit is working properly - ECU diagnostics
