Causes

Short circuit of the control wire to power (+12 V)
Open ground circuit
Second cooling fan relay malfunction
Damage to wiring or connectors
Internal ECU fault

How to Fix

Read OBD-II codes
Check the operation of the second fan when the engine is cold
Measure the voltage in the fan control circuit 2
Check the second fan relay
Check the wire for short to power
