Causes

Short circuit of the control wire of the second fan to ground
Second cooling fan relay malfunction
Broken or damaged wiring
Second fan motor malfunction
Internal ECU fault

How to Fix

Read OBD-II codes
Determine the location of the second cooling fan
Measure the voltage in the fan control circuit 2
Check the second fan relay and its connector
Check fan motor (direct power supply)
If the circuit is working properly - ECU diagnostics
