Causes

Short circuit of the control wire of the third fan to ground
Third fan relay fault
Broken or damaged wiring
Third fan motor malfunction
Internal ECU fault

How to Fix

Read OBD-II codes and determine fan configuration
Measure the voltage in the third fan control circuit
Check the third fan relay
Check fan motor (direct power supply)
Check wiring for shorts and breaks
If the circuit is working properly - ECU diagnostics
